With a physician referral,
the diabetes and nutrition education center will provide an outpatient
program of insulin initiation that offers quality care and convenience.
This outpatient program offers:
Instruction on an individual
or select group basis by a nurse educator and a dietitian
An assessment for diabetes self-management
knowledge and skills and an education plan tailored to individual
needs
A curriculum that offers all 15 content
areas recommended by the American Diabetes Association
Self-blood-glucose-monitor training
and meter verification with a plasma fasting glucose
Self-injection of insulin training with traditional insulin
syringe and/or insulin pen. Insulin adjustments as prescribed
by referring physician
Follow-up written report to your
referring physician
Follow-up individual appointments
with nurse educator or dietitian and/or monthly support groups
Insurance billing through Washington
County Health System
Insurance Information
Plan to have a fasting blood glucose test done at Hagerstown
Medical Laboratory after you have met with the nurse educator
and had your blood glucose meter checked on the first morning.
If you have not had an HbA1c test done within two months, one
will be drawn also. If your insurance requires you to have
blood sent to a special lab, you will need to plan to pay for
the fasting
blood glucose test that is required the morning that we start
insulin. The HbA1c can be done at your insurance lab and the
results
sent to the Diabetes Center.
