The diabetes education team includes the patient's primary care physician or endocrinologist and may include a nurse diabetes clinical specialist who is a certified diabetes educator (CDE), a registered dietitian who is a CDE, a licensed counselor, a registered pharmacist, a registered physical therapist, and registered nurse educators working toward their CDEs. An endocrinologist and a podiatrist serve as advisors to the team and guest lecturers for the group sessions. A board- certified endocrinologist serves as the medical director. Private physicians may refer patients to him for medical consultation and follow-up. A team member assesses each individual's diabetes self-care knowledge, skills, and interests. A plan is developed with the patient for an individualized activity and eating program to match medication, lifestyle, and food preferences to achieve improved blood sugar control and a healthier life. Education is offered on the diabetes disease process; how to prevent, recognize, and treat complications; and skill training in goal setting, blood glucose monitoring, and self-injection of insulin is available, if needed.
