Social work services are available seven days a week and include:

• Patient Assessment to evaluate your personal as well as family and home situations.
• Discharge Planning coordinates the care you will need after you leave the hospital. This may include home health care, medical equipment, nursing home placement, or assisted living facility placement.
• Information & Referral to provide information about community services, such as Meals on Wheels, support groups, and Maternal Child Health services.
• Counseling to listen and help ease your concerns about emotional, social, and financial issues.
• Assistance in completing advance directives, living wills, and durable medical powers of attorney is also available.
