The asthma management program is a free service provided
to children and adults diagnosed with asthma. The purpose of the
program is to provide patient and family education, an asthma symptom
severity assessment, referral to an asthma/allergy specialist if
needed, and to work in collaboration with the patient’s physician
to develop an asthma control plan when symptoms occur.
Through
the
use of peak flow meter monitoring, patients and their families
can monitor daily peak flow readings and take immediate action
when
readings drop or symptoms increase.
A case manager can provide ongoing contact and assessment to
keep
patients and their families up to date with the latest treatments,
recommendations and information about asthma.
