For patients discharged from the hospital with their
intravenous (IV) lines still in place, Home Healthcare offers home
infusion therapy. Under this program, the physician makes a referral
to a pharmacy
that
prepares and delivers the medications to be administered intravenously
on an outpatient basis, and a registered nurse handles the home infusion.
Frequently, new mother and baby are discharged from
the hospital within 24 to 48 hours after delivery (or a few days longer
for C-sections). Home Healthcare’s post partum visit program
provides a skilled nursing visit the day after discharge to evaluate
the newborn’s
physical condition and the mother’s physical and emotional health.
Visits are covered by insurance as required by state law. There is
no charge to the mother or baby for the home care visit.
Home Healthcare’s wound care specialists care for a wide
variety of wounds in the home setting. Working closely with your doctor,
our medical staff will evaluate your condition and develop an individualized
treatment plan designed to put you on the healing path. We’ll
work with you every step of the way, monitoring your progress and making
sure you, your family and/or your caregiver are familiar with your
treatment plan.
Our team of physical and occupational therapists
take pride in helping patients restore capabilities to pre-surgery or pre-accident
levels or reach achievement levels others thought impossible. Our therapists
help patients with challenges related to: joint replacement, motor vehicle
accidents, fractures, stroke and other conditions.
Many health conditions require either ongoing or
intermittent care, either as a follow-up to a hospital visit or to
treat a chronic condition. Home Health Care’s team provides care
for a wide array of needs and conditions including, Congestive Heart
Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Pneumonia,
Diabetes complications, paralysis and other conditions.
The goal of our home psychiatric nursing program is to help the patient
learn to live with a mental illness and enable them to successfully
remain at home. Patients are considered homebound if the nature of
their psychiatric disorder makes them fear leaving home or makes it
unsafe for them to leave home routinely or if another illness or disability
makes it difficult for them to leave home routinely. Once the patient
can consistently leave home safely, care is transferred to an outpatient
setting.
Many patients today have multiple medications to
manage. It seems that with each visit to the doctor, another prescription
is added. Home Healthcare’s medical education program is designed
to ease the multiple medication burden and enable the patient to remain
independent and at home. Working in close communication with the patient’s
doctors and pharmacist, a regisered nurse helps the patient learn to
take medications correctly to prevent possible side effects.
Most families want to do all they can to enable their
loved ones to remain at home as long as possible. When skilled nursing
care
is not required, but assistance
with the activities of daily living is needed, Home Healthcare offers: assistance
with bathing, dressing, taking medications, meals, eating, laundry, light housekeeping,
companionship and more. Our services are generally available seven days per week
between the hours of 8:00 a.m. and 4:30 p.m. Many long term care insurance policies
pay for these services, or Home Health Care can bill monthly. Our nurse
will perform
an in-home interview and assessment and provide you with a written
estimate of all fees in advance.
