Social work services are available seven days a week and include:
Patient Assessment to evaluate your personal as well as family and home situations.
Discharge Planning coordinates the care you will need after you leave the hospital. This may include home health care, medical equipment, nursing home placement, or assisted living facility placement.
Information & Referral to provide information about community services, such as Meals on Wheels, support groups, and Maternal Child Health services.
Counseling to listen and help ease your concerns about emotional, social, and financial issues.
Assistance in completing advance directives, living wills, and durable medical powers of attorney is also available.
