Information for Patients

 

Appointments

For urgent, but non-emergency, situations, we’ll make every effort to ensure that you see your physician promptly. In cases of medical emergency, always call 911.
To make an appointment, please use the online appointment request form or call our office at 240-313-9850. If you need to reschedule or cancel an appointment, we ask that you provide twenty-four (24) hours notice. Because cancellations affect our ability to serve our patients appropriately, we charge $25 for any appointments that are cancelled or rescheduled with less than twenty-four hours notice. This fee is not billable to your insurance company.

Please provide a copy of your insurance card at each visit. Payment is due at the time of service, before you see the physician. This includes co-pays, co-insurance, and deductibles.

If you need care after hours and do not have a medical emergency, consider visiting Urgent Care at Robinwood Medical Center or in the Sylvania Building on Pennsylvania Avenue. If you need help deciding whether to go to the emergency room or to Urgent Care, just call Washington County Hospital’s Advice Line at 1-800-274-0499.

Referrals

If you need to request a referral, you can use the online form or call our office. If you choose to call, please have the following information ready:

  • Insurance plan
  • Physician specialty for which you need a referral (or name of physician, if you know)
  • Whether you have seen the specialist before
  • Date of your appointment with the specialist, if you have already made an appointment
  • Reason for your referral request

Please allow two business days for a referral request to be processed.

Release of Medical Records

In order to release medical records, we need a signed authorization from you. Please allow five business days for the copying of medical records. There may be a copying charge; please contact our office for questions about the cost of copying your medical record.

Privacy Policy

We are committed to protecting our patients’ privacy and never give out personal information without your permission, except as required by law.

We’ll provide you with a detailed explanation of our privacy practices during your first visit; or you can download the Notice of Privacy Practices here.

Additional Information for New Patients

In addition to an insurance card, new patients should bring a photo I.D. to their first appointment with us.

Please also complete and submit the following online form:

  • New Patient Admission Form

Please come to your first appointment about fifteen minutes early. It’s also helpful if you bring your current medication bottles with you to your first appointment, including over-the-counter medicines and nutritional supplements.
